For Children and Teenagers
Being mentally healthy means reaching developmental and emotional milestones, learning healthy social skills, and coping with problems during childhood. Mentally healthy children have a positive quality of life and can function well at home, in school, and in their communities.
Mental disorders among children are described as serious changes in the way children typically learn, behave, or handle their emotions, which causes distress and problems getting through the day. Many children occasionally experience fears and worries or display disruptive behaviours. If symptoms are serious and persistent, they can interfere with school, home, or play activities. This may lead to the child being diagnosed with a mental disorder.
Therefore, it is important to bring your children to a Clinical Psychologist for proper evaluation and management. WQ Park is organizing a project ( 5 free sessions) that can help intervene in issues/challenges faced by your children through expressive art therapy or psychotherapy. Thus by filling up this form, you agree to the services on behalf of your child under 18 years old through this project. Our Clinical Psychologists will provide detailed informed consent when you attend the therapy session with your child.
All personal information data gathered from the Registration Form will remain confidential. Only the WQ Park team and the respective outsource provider are able to access the data of the referred client. It will not be disclosed without the permission of the client. Besides, for the purpose of documentation, the psychological service provider will report the total session the referred client attended as well their brief statement of presenting issues, without compromising its confidentiality.
WQ Park will cover up to 5 (five) psychotherapy session fees for you. If you wish to continue the session with the therapist, the cost will not be covered.
This psychotherapy session is ONLY for Malaysian citizens living in Malaysia. WQ Park reserves the right to change, suspend, or discontinue temporarily or permanently when an individual violates these terms.
